Consent for processing of personal data

Personal Data administrator:

Company ID: 25149628, (hereinafter referred to as "Administrator")

The processing is carried out at its premises, branches and headquarters by individual authorized employees of the administrator. The processing takes place through IT, or. also in manual form for personal data in paper form, in compliance with all security principles for the management and processing of personal data. To this end, the administratorr has taken technical-organizational measures to ensure the protection of personal data, in particular measures to prevent unauthorized or accidental access to, alteration, destruction or loss of personal data, unauthorized transfers, unauthorized processing and other misuse of personal data. All entities to whom personal data may be disclosed respect the right of data subjects to privacy and are obliged to comply with applicable data protection legislation.

The administrator did not appoint a Data Protection Officer.

Legal basis of personal data processing

The legal basis for the processing of your personal data is that it is necessary for:

• performance of a contract between you and the administrator or for the performance of measures by the administrator prior to the conclusion of such a contract within the meaning of Article 6 (1) (a); (b) Regulation 2016/679 of the European Parliament and of the Council on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (the General Data Protection Regulation) ('the Regulation');
• compliance with the legal obligations applicable to the AIFM within the meaning of Article 6 (1) (a); c) of the Regulation, namely the fulfillment of obligations imposed on the administrator by generally binding legal regulations, in particular Act No. 235/2004 Coll., on Value Added Tax, as amended, Act No. 586/1992 Coll., on Taxes and Act No. 563/1991 Coll., on Accounting, as amended.

Text of consent and purpose of processing personal data:

Pursuant to Section 5 (2) of Act No. 101/2000 Coll., On Personal Data Protection, as amended (hereinafter referred to as the "Personal Data Protection Act"), and in accordance with Article 6 (1) (a) | (a) Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation) ), which will take effect on 25 May 2018, I agree to the collection, storage and processing of my personal data provided by me to the administrator.

These data and their purpose of use are:

Name and surname (identification of the person, basic information for differentiation of customers)

title before / after the name (data on education, expertise, optional data)

e-mail (for sending electronic documents, mandatory basic data necessary for communication)

telephone number (fax, mobile) (in case of operative negotiations, consultations, telephone support)

mailing address (for sending ordered goods)

company according to the trade or trade register (data for accounting operations)

business unofficial name (identification of data subject in alternative form, company alias)

Company identification number, tax identification number (identification for accounting operations)

login name (used to log in to the user's account on the administrator's website)

password (used to log in to the user's account on the administrator's website, stored in illegible encrypted form)

Consent will be executed by signing this consent in paper form, or electronically by ticking the box when registering, or additionally by ticking the appropriate consent box in the user's account.

Conclusion

You give your explicit consent to the above processing. The consent can be withdrawn at any time, for example by sending an e-mail or letter to the company contact details,

Please note that under the Personal Data Protection Act and the General Data Protection Regulation you also have the right to:

• to ask us at your request for information about your personal data being processed or confirmation that your data has been deleted; the administrator has the right to reimbursement not exceeding the costs necessary to provide the information, to ask us for an explanation regarding the processing of personal data, to request access to such data and to have it updated or corrected, to withdraw its consent to processing and to whom no other legal reason for the processing of personal data applies, in case of doubt regarding compliance with obligations related to the processing of personal data, contact us or the Office for Personal Data Protection.

Rights of the data subject

Under the conditions set out in the Regulation, you have the right to request from the administrator access to your personal data, the right to rectify or delete your personal data, or limit the processing thereof, the right to object to the processing of your personal data and the right to transfer your personal data. If you believe that the processing of your personal data has violated or violates the Regulation, you have the right, inter alia, to file a complaint with the Supervisory Authority.

You are under no obligation to provide personal information. The provision of your personal data is a necessary requirement for the conclusion and performance of the contract and without the provision of your personal data it is not possible to conclude the contract or fulfill it by the administrator.
